Agus Hariadi, from North Sumatra is accused of throwing his two neighbours and their 12-year-old son into a river near their home in the region on October 8.
Police arrested Hariadi on Monday along with three other men suspected of being his accomplices in the killings.
The four are accused of murdering a 49-year-old man named Muhajir, his 50-year-old wife Suniati and their 12-year-old son M. Solihin.
'The motive is hurt, because he is often ridiculed. The perpetrator was hurt because the victims made fun of his physical appearance,' said North Sumatra Police Chief Inspector General Agus Andriyanto.
